A 37-year-old software engineer presents to his general practitioner with lower back pain that began shortly after New Year’s Eve 2021. The patient reports that the pain started after lifting a heavy suitcase for his mother. Initially, he tried stretching and applying heat, but this did not improve the pain, which he rates as four out of ten. About three weeks later, he sought advice from a pharmacist and started taking ibuprofen 400 mg. The medication helped at first, and he took it intermittently. Several weeks later, however, the pain began radiating to the left thoracic region. Although he then took ibuprofen regularly (3 x 400 mg per day), the pain continued to worsen. In the last five days, he noticed a slight numbness over his hip when sleeping on his side, which made him worry and lead him to consult his physician. At this point, he rates the pain as six out of ten.

A 65-year-old male account manager attends an appointment for follow-up of his known and to-date compensated liver cirrhosis, classified as Child-Pugh A (5 point-score). He developed cirrhosis after an infection with hepatitis C about eight years ago. The patient reports significant stress at work for over one year. During this time, he and his wife felt constantly strained and he lost about 4 kg of body weight without any particular effort. He attributed this to the demanding situation at home together with his workload. Recently, he also developed intermittent upper abdominal pain after meals. Now that his personal life is much better, he can finally take time to look after himself.

A 58-year-old interior designer, residing in both Brazil and Switzerland, presents to his general practitioner in Brazil with persistent upper abdominal pain. He also reports postprandial fullness and mild nausea, despite no changes in the size or composition of his meals. He currently weighs 98 kg and is 174 cm tall. Although he has always struggled with his weight, he recently noticed an unintentional weight loss of 3 kg.
